there used to be a form called a “statutory declaration regarding an assumed name”. My husband used it to apply for a passport in 2010. His birth certificate is in his father’s name, but he has never used that name and has gone by his mother’s last name for his entire life. All of his supporting ID (driver’s license, SIN, healthcare etc etc.) are in his mother’s name. This form allowed him to get a passport using his mother’s name, even though it differed from his birth cert. I am trying to renew our passports which expire next month, and I’m not sure if I need this form again. His current passport is in his assumed name (mother’s name) Does this form no longer exist? I can’t find it anywhere.
The form does not appear to be on the website. You should contact Passport Canada by phone or in person to find out if they can send you a copy.
